If your child has a disability or life-threatening food allergy and you would like a substituted menu that strays from the USDA required meal pattern, you will need to obtain a Disability/Medical Condition Statement for Foods Served by calling the Nurse or Health Associate of the school your child attends, the Nutrition Services Department at (563)744-3885 ext. 6041 or print off the form in the Related Documents box. This form will need to be signed by a physician licensed by the State of Iowa and returned to the nurse where your student attends. If your child's meal conditions can be met by keeping within the set meal pattern, the Disability/Medical Condition Statement form will not be needed, however some form of documentation signed by a licensed practitioner will be needed.
